Evaluate this piece of educational material for readability, cultural competency, and appropriateness for the intended audience. (This is not a discharge form or a consent form. Do not use this document in a teaching session with a patient) . Have a patient and/or lay person (not in healthcare) give you feedback on the usefulness of this educational document.
